What Do EHCP Annual Reviews Look Like at Cann Bridge School?
A Guide for Parents and Carers
Every child or young person with an Education, Health and Care Plan (EHCP) must have their plan reviewed at least once a year. This is a legal (statutory) requirement.
At Cann Bridge School, we aim to make Annual Review meetings collaborative, inclusive, and supportive. We warmly welcome families and all professionals involved with your child to take part, ensuring meaningful conversations that lead to an accurate and up-to-date EHCP.
The Purpose of an Annual Review
The Annual Review ensures that your child’s EHCP continues to reflect who they are now, taking into account changes in their development, strengths, and needs over time.
It provides an important opportunity to:
- Recognise and celebrate your child’s progress and achievements
- Acknowledge the steps taken since the previous review
- Review whether support and provision remain appropriate
- Set realistic yet ambitious next steps for learning and development
The review also plays a key role in planning ahead, helping to prepare your child for their next stage of education and for life beyond school.
Your child remains at the centre of the process at all times. The views of families, the child or young person, and all professionals are valued to ensure shared understanding and collaborative planning.
Preparing for the Review Meeting
- Reviews are planned across the school year by the EHCP Annual Review Coordinator
- You will receive an invitation at least 6 weeks in advance
- You will be asked to complete a Parent Contribution Form and Parent Views Form
- The class teacher prepares a draft review, including progress towards EHCP long-term outcomes
- Your child is supported to share their Learner Views and Learner Contribution Form in a way that suits them
- All relevant professionals are invited to attend or submit a report
- You will have the opportunity to see draft paperwork before the meeting—nothing should come as a surprise
The Annual Review Meeting
- Your child may attend all or part of the meeting, where appropriate
- The meeting is usually chaired by your child’s class teacher
- The atmosphere is calm, respectful, and supportive
During the meeting:
- The EHCP is reviewed section by section to check accuracy
- Progress towards outcomes is shared and discussed
- Any changes to outcomes or provision are agreed collaboratively
- Actions for school staff and other professionals are recorded
- There is time for questions, discussion, and sharing your views as a family
After the Meeting

The EHCP Annual Review Coordinator sends all required paperwork to the Local Authority within 2 weeks

The Local Authority will decide whether to:
- Maintain the EHCP as it stands, or
- Issue an amended EHCP

Amendments are typically made at key transition points (e.g. Year 6 and Year 11) or where a change of placement is being considered

Parents/carers will be formally notified of the decision

You will have the opportunity to comment on or challenge the draft document within 2 weeks of receiving it from the Local Authority
School Roles and Responsibilities
EHCP Annual Review Coordinator
- Organises review dates and meetings
- Sends invitations and paperwork to families and professionals
- Ensures all paperwork is completed and submitted on time
Class Teacher
- Reviews and suggests update to each section of the EHCP
- Provides information on progress and outcomes
- Gathers Learner Views and Learner Contributions
- Chairs the Annual Review meeting
All School Staff
- Implement agreed support and provision
- Monitor progress of the EHCP Long Term Outcomes through writing, deliverying and assessing termly, Personalised Learning Goals (PLGs)
Parent and Carer Responsibilities
We ask parents and carers to:
- Attend the Annual Review meeting (in person or virtually, if requested)
- Complete and return Parent Contribution Form and Parent Views Form
- Share any changes in health, home circumstances, or your child’s needs
- Work in partnership with school staff to support your child’s progress
- Work towards EHCP Long Term Outcomes at home and transfer learning from school to home, and home to school.
Your involvement helps ensure the EHCP provides a true and up-to-date picture of your child.
Reviewing, Tracking and Setting EHCP Outcomes
- EHCP outcomes are long-term goals, usually set to be achieved by the end of a Key Stage
- Outcomes are reviewed and closed when achieved
- All outcomes are written as SMART targets (Specific, Measurable, Achievable, Relevant, Time-bound)
To support progress:
- Outcomes are broken down into smaller steps called Personalised Learning Goals (PLGs)
- PLGs are reviewed termly and shared with parents/carers so that they can also be worked towards at home
EHCP structure:
- Section B identifies needs
- Section E outlines outcomes linked to those needs
- Section F details the support and provision required
From Year 9 onwards, outcomes focus on Preparing for Adulthood, ensuring clear pathways into later life.
Interim Review Meetings
An Interim Review may take place where important changes need to be considered before the Annual Review, such as:
- Significant changes in circumstances
- A proposed change of placement
- A request for changes to funding or provision
