Cann Bridge School SEND Family Hub

A welcoming local hub for children, young people with SEND, and their families

Cann Bridge School is proud to be a SEND Family Hub Satellite Site, working in partnership with Plymouth Family Hub (Barnardo’s – Whitleigh). Together, we are creating a welcoming, accessible space where families can find early help, advice, and joined‑up support close to home.

This partnership strengthens support for children and young people aged 0–19 (or up to 25 with special educational needs and disabilities), ensuring families don’t have to navigate services alone.


What is the SEND Family Hub at Cann Bridge?

The Cann Bridge SEND Family Hub is part of Plymouth’s city‑wide Family Hub network. As a satellite site, it brings services directly into the heart of the community, reducing travel barriers and making support easier to access for families who may otherwise find it difficult.

The hub is designed to be:

  • Warm, inclusive and family‑friendly
  • Focused on early help and prevention
  • Accessible for children and young people with complex needs
  • Connected to health, education, and family support services

Over time, as local needs develop, the site may expand its offer further in partnership with the Local Authority.


What support can families access?

Services at the Cann Bridge SEND Family Hub may vary depending on local need, but can include:

  • Support with routines, behaviour, and emotional wellbeing
  • Opportunities to share experiences with other families

🧒 Early Years and Child Development

  • Visiting specialist services where appropriate

📍 Information, Advice and Navigation

  • A local information and advice point for SEND families
  • Clear signposting to specialist support and main Family Hubs
  • Help navigating services using the “Tell Your Story Once” approach

How the Hub works

  • Services may run on a flexible or needs‑led basis, rather than a fixed timetable
  • Families can be supported directly on site or linked seamlessly to the main Whitleigh Family Hub
  • A named Family Hub link worker is connected to Cann Bridge School, ensuring joined‑up communication and consistent support

Cann Bridge staff are trained in Family Hub principles and early help, enabling them to confidently support, guide, and signpost families.
